DISCOUNT REHAB MEMBERSHIP PLAN

The ABC Medical Group & The SideDor Group are proud to offer a Discount Rehab Membership Plan to students and adults who don’t have medical insurance or those with high deductibles and/or lapse in coverage. With proof of membership a member can be seen at any ABC Medical Facility at a special reduced membership rate.

Benefits – As a member of the discount plan, the member will receive office visits and procedures as provided by the ABC Medical Group at a discount rate.

Membership – The Discount Rehab Membership Plan is NOT insurance or a workers compensation plan; it is a discount plan that can be used for services provided by the ABC Medical Group.

Membership cost - $20.00 per month (min 1 year subscription with a one-time $5.00 registration fee.)

ABC Medical Group Services:

· Physical therapy/Chiropractic therapy (Sports Injury Rehab) – Student-$10.00 per visit / Adult-$25.00 per visit. Upon medical referral members receive Physical therapy/Chiropractic therapy as recommended.

· X-rays - $15.00 per 1 view. (Note – the physician may require multiple views).

· Medical office visit, assessment and recommendations - Student-$30.00 / Adult-$40.00 per visit. A medical physician and or certified physician assistant will evaluate the member. Upon medical assessment, the medical physician will provide the appropriate treatment (medication, X-rays) or referral (physical therapy or orthopedic referral).

· Orthopedic evaluation and assessment - Student-$50.00 / Adult-$100.00 per visit. The Orthopedic specialist will provide orthopedic assessment and recommendations.

Additional Discounts:

· Discount rate on medication in stock provided by the ABC Medical Group. (Note – a prescription may also be provided for medication that the ABC Medical Group does not provide.)

· Physicals – Students-$25.00 / Adult-$40.00.

Proof of Membership – Members must present their Membership Identification Card, along with valid photo identification at the time medical service is rendered.

Cancellation Policy – The Discount Rehab Membership Plan may be cancelled at any time with a 30-day notice without any further obligation. $75 cancellation fee will apply.
